Assistant (CNA) or Home Health Aide (HHA) dedicated to providing exceptional in-home care for clients in the Palm Beach County Area. You will be responsible for delivering high-quality patient care while promoting independence and comfort in the familiar setting of each client’s home. Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support.Support clients with Alzheimer's care, dementia care, memory care, to ensure comfort and safety. Utilize Electronic Health Record (EHR) systems to document medical records, care plans, and progress notes precisely. (Wellsky/Kinnser preferred) Operate medical equipment such as Hoyer lifts and perform medical documentation with attention to detail. Conduct home safety assessments, light cleaning for patient area, laundry, meal preparation, and ensure a clean, safe environment for clients. Maintain confidentiality and adhere to all HIPAA guidelines while providing patient-centered care during home visits. Skills Strong patient observation skills coupled with the ability to recognize changes in health status and reporting promptly to supervisors Experience working in assisted living facilities, nursing homes, or home health environments is highly valued. Knowledge of dementia care techniques, and Alzheimer's support strategies. Ability to assist with ADLs effectively while demonstrating compassion and patience. Certifications such as CPR are required, as well as 12 continuing education hours yearly. Excellent communication skills to collaborate with healthcare teams and educate patients on their care routines. Must be fluent in written and spoken English. Capable of performing light housekeeping tasks including cleaning patient areas to maintain a healthy environment.
